Due
to the large volume of insurance plans that are available, Women's
International Pharmacy cannot guarantee specific coverage for a
particular prescription. It is always recommended that the patient
check into their specific policy as to what the limitations are
on their policy.
The Patient Information Sheet that comes with each
prescription can be used to file claims with your insurance company.
Please note, your insurance company will pay you directly. You are
responsible for your account and it should be paid in full monthly to avoid
delay in services. |

|
Your practitioner may have written your prescription for a 60 or 90
day supply of medication. Depending on your insurance company and/or state
regulations, you may be limited to a 1 month supply. It is customary
for Women's International Pharmacy to limit first time customers
to a one month supply.

|
Customers
may pay by check, by credit card
on-line, or by providing Women's International Pharmacy with a credit card number written on the
lower portion of the invoice
stub, or the remittance portion at the top of the statement.
Upon request, we will keep your credit card number on file for future
orders.

|
Women's International Pharmacy, Inc.'s
billing cycle closes at the end of the month, therefore,
your payment may not have been received or credited to your account
prior to processing and mailing of statements.
