Each
prescription will be accompanied with a Patient Information Sheet and an
Invoice.
The Patient
Information Sheet is included with each prescription and/or supplement to
reflect the cost. This Patient Information Sheet may be used for insurance and/or income tax purposes and does
not include the shipping and handling charge.
The Invoice is
also included to verify the cost of each item ordered, the total items
shipped, and the shipping and handling charge. Upon receipt of your order,
please pay total amount due as shown on the Invoice.
Please note
that payments received after the last day of the month, will be reflected on
your next statement.
